The checklist form is available in two formats:

  • A PDF form that can be downloaded for your use and can be reproduced and distributed.
  • An on-line form. It anonymously adds your data to our database for benchmarking.

We invite you to participate in the on-line checklist to build industry benchmarking information and make your plans for continuous improvement.
